UNDER 18’s PLAYER REGISTRATION FORM

A Note for Parents and Carers
When registering a player under 18, the form must be completed by their parent or carer.

This ensures we have the correct details for both the player and their legal guardian, in line with The FA’s safeguarding rules.
It also means all club communication will go through the parent/carer, not directly to the player.

By filling out these sections, you’re giving consent for your child to play football with the club and helping us provide a safe and supportive environment for every young player.
